My knowledge in this area is nearly indistinguishable from zero, so take this with a grain of salt, but:

I seem to recall hearing about problems like this related to phone unlocking. I recall that some third-party unlock hacks would reset the push notification token. I believe there was one a while ago which just gave everyone who used it the same token, leading to much pushnote-related hilarity. Perhaps this is what you're running into, on a smaller scale somehow? Worth consideration, perhaps.
